Apple said that a permanent or temporary change of the skin, such as tattoos, will affect the performance of the heart rate sensor. Ink and Tattoo use may cause the sensor can not be sensitive to light, so read the information unreliable.
On sites like Reddit and Twitter, Apple Watch user posted some conflicting reports and videos. Some have shown that even the presence of a tattoo, heart rate tracking can still be used normally. But others show that heart rate tracking information can not be read, or read the wrong information.
This problem could have major impact of the tattoo darker areas, such as areas of the black and red patterns.
If you really can not read the heart rate information, but you also want to use Apple Watch to track the effect of exercise, then Apple's technical support documents recommend that the user use a separate Bluetooth connection with a chest strap to track heart rate.
